Request a certificate of Insurance
This is for policy holders with current policies. If you do not have a policy, please start by filling out an application.
In order to bind a policy, payment is required. Annual DICE and annual event programs may be financeable upon underwriting approval. All other programs such as short term, E&O, etc. are required to be paid in full. Short term policy premiums are immediately earned and non refundable as per the insurance carriers.
Acceptable payment methods are cash, certified or cahiers checks, regular check (please provide an extra day or two for this option for funds to clear), credit cards or wire transfer (please request account info). If you are paying with a credit card, click here for a Credit Card Authorization form. Coverage cannot be cancelled nor amended once bound.
For any other type of incident in which a claim might be filed, please contact us at your earliest convenience, during business hours (Mon-Fri 9AM-6PM EDT).
Please be prepared to provide as much information as available, including, but not limited to:
•A Detailed Narrative of the Incident
•Police or Fire Department Reports
•Summons, Complaints or Lawsuits
•Contracts, Agreements or Releases
